If you haven't attended our week-long camp in the past, no worries!!! Here is an opportunity for you to get the experience that everyone has been talking about. Everyone will be attending camp to get a full 4 hours of soccer to push our campers to become better players. Whether you are new to the game or you have experience this is a perfect opportunity for you to experience what Trejo Soccer Academy can offer you.

Emergency Contact

Trejo Soccer Academy is not responsible for any previous injury or recurrence of any injury. I will not hold Trejo Soccer Academy, instructors, or Forest City Parks and Rec. for any claims of personal illness or injury that my child may sustain during camp.

Authorization for Emergency Treatment: I hereby give permission for the minor named above to receive emergency medical treatment, in the event of an emergency that I/we cannot be reached. I understand that this will be at my expense.

Photography Release: I hereby authorize TSA to publish photographs taken (for the year 2023) of myself and/or the minor child or children listed, for use in the (TSA Camp)'s print, online and video-based materials, as well as other camp publications.
